Witmore Enterprises Inc. Transisition to Work Program

• Vocational and Life Skills development from school to the workforce
• Certificate 1 - Work Skills
• CPR and Basic First Aid Skills
• Supported Work Experience
• Two (2) year program
• Clients will attend three (3) days per week
• Operating hours are 9am to 3pm Monday to Friday

Process

• ADHC conducts an assessment of client while they are still attending school
  
• ADHC notifies client of outcome and advises suitable program
  
• Client contacts Witmore to enter and commence program
  
• Witmore will assist the client in developing a Person Centred Plan (PCP) which will outline their goals and aspirations
  
• Witmore will provide support to the client to enable them to achieve the outcomes of the PCP
  
• The PCP is reviewed regularly with the client and changes are made as required
  
• ADHC will conduct a further assessment of the client at the expiration of the two (2) years.
  
• This assessment will determine the most appropriate pathway for the client to follow, either Disability Employment Net Work or Community Participation Program
